SMS opt-in on the Calendar Compact booking form

This is the same consent control customers see when they enter a mobile number to finish a booking. Phone and email are required so the operator can reach them. The box starts unchecked. Checking it is how you agree to receive automated SMS from Calendar Compact. You can finish the booking without checking it — then we email only.
